Rowdy’s Youth Academy Release Form & Liability Waiver
By completing the following forms, I:
In consideration of my child’s participation in the Montgomery County Constable Pct. 4 – Rowdy’s Youth Academy, permit any image, voice, and participation to be recorded and shared on social media platforms. I understand that these materials may be used publicly and agree to their use for informational or promotional purposes.
I assume all risks associated with my child’s participation in the Montgomery County Constable Pct. 4 – Rowdy’s Youth Academy and hereby release and hold harmless the County of Montgomery and Montgomery County Constable Pct. 4 Office.
Understand that Montgomery County Constable Pct. 4, Constable Kenneth Hayden and his Deputies, nor the County of Montgomery, are responsible for any injuries or harm while participating/volunteering in Rowdy’s Youth Academy. I furthermore affirm that my child is physically able to participate in the Montgomery County Constable Pct. 4 – Rowdy’s Youth Academy.
My child will comply with all rules of the Montgomery County Constable Pct. 4 – Rowdy’s Youth Academy and will professionally conduct themselves at all times.
I am aware that my child’s participation can be revoked if I/they do not comply with the rules set by the Montgomery County Constable Pct. 4 – Rowdy’s Youth Academy.
My child will be dropped off no later than 9am and picked up no later than 4pm.
